Introduction

Welcome to Sexual Health and HIV Prevention in the State of Colorado, an activity based on a live symposium presented at the Colorado Academy of Family Physicians (CAFP) Annual Summit on April 22, 2017. This case-based learning activity is designed to help you (1) assess patients and identify those who might benefit from HIV pre-exposure prophylaxis (PrEP); (2) administer PrEP appropriately; and (3) provide the support your patients need to effectively prevent HIV.

We recognize that prescribing PrEP may be a new competency for your practice and that you may not have a medical home built around HIV prevention. For this reason, this site also provides links to resources and services in your region of Colorado to support you in your efforts to care for your at-risk patients. You will find established PrEP providers; resources from our partner, the Colorado Health Network; guidelines for PrEP; as well as educational opportunities specific to your area. Thank you for joining us to learn more about this important topic and for your commitment to your patients at risk for HIV.

Learning objectives

After completing this activity, the participant should be better able to:

  • Conduct an effective sexual health assessment
  • Identify groups at sufficiently high risk for HIV to warrant consideration of PrEP
  • Evaluate the indications and appropriate prescribing/monitoring practices for the PrEP regimen
  • Provide PrEP or refer patients at sufficient risk for the PrEP regimen
  • Connect patients at risk for HIV with regional patient support resources

Build a Medical Home Around Your Patients At-risk for HIV

Thank you for participating in this activity, Sexual Health and HIV Prevention in the State of Colorado. Part of our effort, beyond educating you on the clinical aspects of HIV prevention, is directed to connecting your patients with ancillary services provided by the Colorado Health Network (CHN). Below is a sheet to hand to your patient with the contact information for the CHN regional prevention manager as well as other information about the CHN. We have also included a link to a survey the patient can take to ascertain the quality of their experience with the prevention services from your office and the CHN. We will share aggregated results with you regarding the impact of the initiative.
